These prepared forms act as an information sheet for the proposed treatment. They can be very useful as a patient education tool but must be part of a detailed conversation with the patient. If the form alone is used, the consent may be effective, but the risk management benefit of informed consent will be lost because the physician will not learn about the patient’s special concerns.
