TITLE:  DoD Instruction 6055.7, "Accident Investigation, Reporting, and Record Keeping", 10/03/2000

SUMMARY: This Instruction: (1) reissues and updates DoD Instruction 6055.7, December 16, 1981. (2) Incorporates provisions of references Defense Safety and Occupational health Program Policy Memorandum (SOHPPM 84-2), SOHPPM 84-3, SOHPPM 84-4, and SOHPPM 84-5 pursuant to DoD Directive 1000.3 to update guidance for investigation, reporting, and recordkeeping on mishaps and occupational illnesses. (3) Provides format and instructions for preparing reports to fulfill the needs generated by SOHPPM 84-5, DoD Directive 1000.3, E.O. 12196 and Occupational Safety and Health Administration Regulation, "Basic Program Elements for Federal Employees Occupational Safety and Health program," October 21, 1980 (29 CFR part 1960) and DoD 6055.9-STD. (4) Centralizes the collection, analysis, and reporting of DoD fire loss data. (5) Continues the authorization to publish DoD 6055.7-M to standardize existing fire reporting systems of the Military Departments and the Defense Logistics Agency (DLA), and to prescribe reports compatible with the requirements of the Federal Emergency Management Agency (FEMA). (60 provides a catalog of charts for DoD program review data displays on aviation and ground safety statistics and common definitions of military aircraft categories.
