Introduction

The starting point for all medical legal inquiries is the medical record. Many physicians have come to regard the medical record as a legal document, kept for reasons separate from patient care. Discussions about medical malpractice usually portray the good medical record as a totem to ward off litigation. Good medical practice is the only cure for malpractice litigation. Medical records are good only to the extent that they facilitate quality medical care.
